Contact Form Example

This page shows the Contact Form in action.  You can easily define as many fields as you wish, what type of fields they are, and how they are ranked.

Each contact form on your website can be emailed to a different email addresses.  For example, create a general contact form that will get mailed to your info@domain.com address but create surveys to be sent to another person in your organization.
